〜上で is a formal piece of grammar that links two actions and has two main usages: "upon doing X,... Y", or "while doing X,... Y".
〜上で can attach to casual past/present form verbs, or nouns using the の particle.
This entry concerns the "upon doing X,... Y" usage:
E.g. この説明書をよく読んだ上で、ご利用ください。 "Please use (this) upon having carefully read the instructions."
E.g. 家族と相談した上で返事をします。 "I will reply upon conferring with my family."
E.g. 週末は混むので、ご予約の上、お越しください。 "The weekend will be crowded, so please make a reservation prior to visiting."
Notes:

The subject of both actions in a 〜上で sentence should be the same.

上で should not be used to link together two actions that have an obvious, or unchangeable order.
